The Ugandan Ministry of Health has reactivated its Traveller Health in light of the recent Ebola Bundibugyo virus outbreak.
On its Point of Entry Self Declaration portal, the ministry states that the form is designed to support health screening measures and help prevent the cross-border spread of Ebola and other disease outbreaks by monitoring travellers entering and leaving Uganda.
The form requires the following information:
- Personal traveller information
- Travel details including date of entry, port of entry, flight or vehicle number and seat number
- Address and duration of stay in Uganda
- Contact tracing information
The form should be completed before departure. Once it is submitted, travellers will receive a confirmation message or reference code which may be requested by officials upon arrival in Uganda.
Health screening procedures may still apply at Entebbe International Airport if required by authorities.
