India mandates health declarations

India now requires all international passengers to complete a Self-Declaration Form (SDF) before arriving in the country.

The form requires travellers to complete a 21-day travel history, including any visits to Ebola-affected countries. 

Travellers must complete the SDF before departure and present a printed copy at immigration on arrival at their first airport of entry. 

Travellers are advised to read the FAQs before completing the form. The SDF must be submitted via the online portal

Passengers departing India are not required to complete the SDF.
