All travellers entering Tanzania are now required to complete an online health declaration form. The form is available at https://afyamsafiri.moh.go.tz and must be completed before departure or on arrival.
According to Tanzania’s Ministry of Health, travellers must provide accurate information and retain their confirmation QR code or reference number for verification by port health authorities on arrival. The measures were outlined in Travel Advisory No. 18, issued by the Government of Tanzania on October 17.
The additional measures are in response to the Ebola Virus Disease (EVD) outbreak in the Democratic Republic of Congo (DRC).
Previously, the declaration was required only for travellers arriving from or transiting through the DRC or Uganda, but has now been extended to all travellers.
Health screening, including temperature checks, will also be conducted at all points of entry, including airports, land border crossings and seaports.
Zanzibar’s Ministry of Health published its own Ebola-related travel advisory on October 9. All travellers arriving from or transiting through the DRC, Uganda or Kenya are required to complete the Traveler's Health Surveillance Form at: healthtravelznz.mohz.go.tz.
All arriving international passengers will undergo public health screening at Zanzibar's designated international airports and seaports.
